This is for a Dept 56 Snow Village Oak Grove Tudor house from 1991 that includes the light cord (but not a lightbulb), model number 5400-3. The box has long since disappeared, probably because it was on display year-round for a number of years in the 1990's and early 2000's. It did have an accident at one point and the front left portion of the house has a repair that is very hard to see when not looking for it. It's been sitting in climate-controlled storage for the past 15-20 years. The pictures show where it was broken and repaired.
